Title: Survey Interview Form and Parent/Caregiver Rating Form from the series Vineland-II Vineland Adaptive Behavior Scales (a revision of the Vineland Social Maturity Scale). Clinical instrument measure personal and social skills needed for everyday living. Age range is birth to age 90. Forms aid in diagnosing and classifying mental retardation and other disorders such as autism, Asperger Syndrome, and developmental delays. Eleven Vineland-II subdomains are grouped into four domain composites: Communication, Daily Living Skills, Socialization, and Motor Skills. Motor Skills domain is used only with ages birth through 6 years. Survey Interview Form provides a targeted assessment of adaptive behavior and uses a semistructured interview format to gather information from a parent or caregiver. The Parent/Caregiver Rating Form covers the same content as the Survey Interview, but uses a rating scale format. Administration time is 20-60 minutes.
